ADHD & Autistic Spectrum Conditions

Autism (ASC) & Neurodiversity 

Children (<18) 

You do not usually need to contact the GP practice for an autism assessment referral.

Referrals can be made directly to Children and Family Health Devon (CFHD), and schools or educational settings are often best placed to support this process.

Before contacting the practice, please ensure you have:
– Reviewed the CFHD website guidance and videos
– Completed all required referral documentation

CFHD provides comprehensive resources and guidance for parents and carers. 

Adults (>18) 

All adult autism referrals to DAANA must come via the GP practice.

To avoid delays, you must:
– Complete the referral form fully and accurately before submitting it to us
– Return the completed form for review and submission

Incomplete forms will not be processed and will be returned.

Right to Choose:
– You must choose your provider yourself
– You must complete all provider-specific forms
– We cannot choose a provider or complete forms on your behalf

If you choose a non-approved provider, this may not be funded by the NHS.

Where shared care is not in place, the provider remains responsible for prescribing medication. 

ADHD 

Children (<18) 

ADHD referrals must be made via the GP with supporting documentation.

You must provide:
– Completed parent/carer forms
– Completed education forms

Referrals for children aged 6 and under will not be accepted.
If there are significant mental health concerns, a CAMHS referral may be more appropriate. 

It is our policy at ISCA Medical Practice that we do not undertake ADHD prescribing for children.

Adults (>18) 

Adult ADHD referrals to DAANA must come via the GP practice.

You must complete the referral form fully before submission.
Forms that are incomplete or incorrect will be returned.

Right to Choose:
– You must select an approved provider
– You must complete all required documentation
– We cannot recommend or complete forms

Where shared care is not agreed, the provider must prescribe medication.

  • It is your responsibility to ensure all forms are completed correctly
  • Incomplete or incorrect submissions will be returned and will delay your referral
  • We are unable to complete or amend forms on your behalf
  • We cannot progress referrals without the required documentation

Due to the high volume of requests, processing times may vary and urgent clinical work will always be prioritised
