Prescription Targets
- 100% of Acute prescriptions (medicine prescribed by the clinical team during an appointment) will be to the nominated pharmacy within an hour.
- Any queries raised by patients about acute prescriptions or queries from community pharmacies to be dealt with on the day by the duty doctor.
- Repeat scripts (medication on repeat prescription) – 95% will be sent to the nominated pharmacy within 3 working days from receiving request and 100% within 5 working days, providing the patient is up to date with essential monitoring.
- Patients will be contacted by a doctor, member of the pharmacy team or administration team within 2 working days if prescription cannot be issued or there is a query.
- Once a query has been addressed and a prescription is appropriate, 100% of prescriptions will be send to the pharmacy by the end of the next working day.
- Scripts for medication suggested by our nurses, PCN team or in-house smoking cessation team will be sent to the nominated pharmacy within 48hrs if clinically appropriate. If not appropriate, patients will be informed within 48hrs.
- Hospital contracts require the hospital to provide a short supply of urgent medications. New medication(s) which have been started by secondary care will be updated on repeats within 14 days of receiving the notification, if clinically appropriate.
- Decisions on shared care prescribing will be made within 14 days. Until they have been approved responsibility for prescribing remains with the specialist.
