The Place

The Isca Medical Practice is a general medical practice operating from two buildings in Exeter. The antecedent Practices had been there, or in the building that was destroyed by a bomb, for decades.

The Business

The Practice is a partnership, of 4 doctors who are Principals in general practice and who jointly own the business. The buildings are owned by GPs, not by the State or the NHS. The large majority of work is done under a contract with the NHS, but other activities also contribute to the business income, out of which come all salaries to the staff and drawings of the partners. Some of these are the provision of a restricted range of services the NHS does not purchase to NHS patients of the Practice, and some are regular contracted services to other local businesses, ad hoc jobs and other charageable work outside by the partners and staff.

The business of the Practice is the business of the Partners, and the business of the Partners is whatever the Partners decide is the business of the Partners.

Staff are employed by the Partners, paid by the partners, and work for the Partners. Non-clinical staff report to the Practice Manager, who reports to the Partners. The only clinical staff currently employed are nurses, who report to the chief nurse. The organisation generally runs on a relaxed and informal basis, with everyone making their best efforts rather than by detailed supervision and instruction.

The Jobs

The jobs done and people doing them are generally those found in small general medical practices in urban areas. There is less differentiation of roles than in larger organisations, and the partners do not currently have individual personal assistants or secretaries.

There are several sorts of attached or associated staff employed by the NHS and paid and working for the NHS, including District Nurses, Physiotherapists and midwives who are regularly present. Health visitors have been seen. It is necessary, desirable, and valued by the partners that the Practice maintains good working relations with all of these.
